Hospice is responsible for the whole person, responsible for the Family and the Caregiver and we have a broad team of people that enhances everyone lives and create an experience that helps everyone. As part of that team, you the RN you are responsible for the clinical part of the whole. We have two Medical Directors that are actively involved with Pathway and with the patients to ensure that the patient and the family experience the best possible care. Our Medical Directors oversee the clinical care and will provide input and direction, but it is the Registered Nurse who is the "boots on the ground" so you are responsible to collect and share meaningful information, and assess the changing conditions. As a primary player in the Interdisciplinary group, you help determine and execute the plan of care.

You are responsible to attend to the patient as scheduled and as needed beyond the schedule. You, the RN, are the one, when it comes to emergent issues. Falls, changes in condition, and deaths, You will be the one the patient, the family, Pathway and Medicare rely on to be responsive and effective. There is no greater reward as a nurse than being appreciated and recognized for your efforts and abilities.

Chaplain

There are few more meaningful and emotional times in anyone's life than when they are considering what happens at the end of it.

And there are few callings in life that can be more gratifying than being there for someone how is on that path. Hospice Chaplaincy is a special opportunity, and with Pathway it is a great way to find out if you would get the life affirming experiences that add meaning and purpose to our own lives.

This would be a part time, on your time, as slowly as you want, position. This is a Paid Position, unless you want it to be volunteer.

Every person is unique, and Pathway is dedicated to providing the types and levels of care the patient and family want, as they want them. Not everyone is open to that exploration on day 1. As opportunity arise, as people show some interest in exploring the last chapter of their mortal existence, you can decide if you are ready and willing to be there for them. We have Staff Chaplains that will take this on if it not right for you. So we have your back. Your involvement with the patient (and possibly their loved ones) might be a brief encounter, or it could be a longer-term relationship. It might be regularly scheduled, or it might be intermittent. Pathway's goal is the best patient's experience, if you have that goal, we can do great things.

A successful Hospice Chaplain is someone who listens, not someone who tells. The End-Of-Life journey is one of self-discovery, not a programmed tour with a scripted guide. You don't need to know a lot; you need to be available and willing to share and allow the other person to dictate the conversation. We all have a different accumulation of experiences and thoughts that are the foundation of our own belief system. Good Chaplaincy is helping someone find answers from within. There will be many that may rely on you and your understanding, to find their own footing and start to plan and understand.

Can you be emotionally available? Can you support and encourage that person to believe in their own way?

Hospice Specialist Nurse

As Southern Utah's Only Hospice Specialist, Pathway Hospice and Palliative Care, brings a wide range of additional palliative modalities and far more care that enhances the patient experience. 
In Hospice, the RN is a critical link in the continuum of patient care. You investigate and assess the patient's conditions, changes and needs; report them to the IDG team which includes the Medical Directors, Director of Nursing, Administrator, and other disciplines so we can create a coordinated care plan to effectively manage all aspects of the patient's condition, issues and life, along with managing and caring for the family and caregiver.

Hospice is unlike any other part of medicine, the disease or condition is not fixable, there are no heroic measures that will change that, we try to help the patient and family redirect their focus on the things they can control; on gaining the best outcome.

Hospice done right is an amazing experience. We have those experiences all the time.
