We don’t need all this information for us to reach out, but the more information we have to start, the more effective we can be finding resources and support quickly.
Give us your name, so we can let them know why we are contacting them.
We’ll need contact information for the person, or their loved one that cares for them.
We’ll reach out to them, answer their questions, help them to better understand what options and choices they have, and then if appropriate we will set an appointment and visit them in person to find ways that we can make their life better, for as long as possible.
There is no commitment, no cost, no obligation for you, or for whomever you suggest we speak with. AND, if our services are useful, there is no cost to them, no deductible, no co-pay.
It may be that today is not the best day to start, but if we can communicate, at least they know that they have options in the future.
As a Medicare provider we are fully HIPAA compliant, this limits and controls our access to and ability to share your personal medical information.
